Our 24/7, multi-lingual claims department offers translation services, claims management and settlement processing, online claims submission, and medical claims assistance through Active Care Management (ACM).

To file a claim, please follow the instructions below:


Include copies of all documents, invoices, and receipts related to your claim. Pictures of receipts taken from your smartphone are accepted. In some cases you may be required to submit original copies or receipts.


Return the form and all documents related to your claim by one of the following:

  • Email – Save your completed claim form, attach any other documents related to your claim and send it to
  • Mail – ACM P.O. Box 1237 Station A, Windsor, ON N9A 6P8
  • Fax – 519-251-5165
